What is it for?
An accident book helps the event lead keep track of what happened, where the detailed report is stored, and which actions still need attention. Use this register alongside a separate accident report for each incident.
Use it at community days, holiday clubs, fundraising events and volunteer sessions. The index uses report references rather than names or medical details, so it is easier to share action tracking without sharing the full incident record.
How to use this template
- Appoint a record keeper and choose a secure location for individual reports.
- Assign each incident a unique reference and complete the separate accident report form.
- Enter the reference, date, location and action owner in this register.
- Review outstanding actions and record who checked any reporting duties.

Make it your own
Open the Excel workbook in Excel or import it into Google Sheets. Read the setup notes, then adapt the rows to your event. The CSV is a plain data version; the PDF is laid out for printing.
