Photo consent form

What is it for?

Use this form when your organisation has decided consent is the appropriate basis for a specific use of photographs or video. Explain the purpose and channels in advance and make it easy to say no.

Suitable for a named event and clearly described uses such as an organisation’s website, printed newsletter or social accounts. Obtain the appropriate permission for children and discuss their own wishes. Choose an appropriate process for adults who may need support with the decision.

How to use this template

1. Describe exactly what will be recorded, why, and where it may appear.
2. Fill in the use period, privacy notice and withdrawal contact before sharing.
3. Ask for a separate Yes or No for each channel; leave no option preselected.
4. Brief the photographer privately and keep consent records linked to the relevant images.
