What is it for?

A one-page profile introduces a participant as a person: their strengths, what matters to them, and the support that helps them enjoy an event. Complete it with the person and use their own words wherever possible.

Use before a holiday club, community session or supported activity. Share only the agreed information with the people who need it. It is a short introduction, not a care plan, clinical assessment or emergency medical record.

How to use this template

  1. Invite the person to choose what they want the team to know.
  2. Describe strengths and interests as well as practical support preferences.
  3. Agree who can see the profile and when to review it.
  4. Brief the relevant team privately and update the profile when preferences change.
