Volunteer self-declaration form

What is it for?

Give volunteers a clear way to confirm the information they have supplied and acknowledge the event’s role guidance. This is a general participation and conduct declaration that you can adapt to your organisation.

Use after sharing the role description, code of conduct and reporting route. It deliberately does not ask for criminal records, health histories or a broad declaration of fitness. Those require a separate, properly scoped process where necessary.

How to use this template

1. Add the event, role and named contact, and supply the documents listed on the form.
2. Ask the volunteer to read the statements and raise anything they do not understand.
3. Let them request a private conversation about support or adjustments.
4. Record their confirmation and complete any separate recruitment checks needed for the role.
